Case Management

 

 AOP Consult case management approach utilizes a collaborative process of assessment, facilitation, care coordination, person centered service planning, evaluation and advocacy services to meet participants comprehensive health and concrete services needs thereby promoting safety, quality of care and cost effective outcomes.

Clinical Assessment

 

The bedrock of any intervention is to identify and diagnose appropriately. At AOP Consult, we support core service agencies, skilled nursing facilities, mental health agencies and social services organizations by providing clinical and psychosocial assessments. Our licensed social workers are vast with the use of the latest version of the Diagnostic and Statistical Manual of Mental Disorders ( DSM -5 ) and well skilled at obtaining psycho-social information impacting a person’s overall well being.  

Our clinical / psychosocial assessment goal is to provide you with the most accurate information on the client so that you can focus on the medication and/ or psychotherapeutic support needed. 


Monitoring & Evaluation

 

Social Services agencies are tasked with the responsibility of identifying the most valuable and efficient use of limited resources. Our approach collects and analyzes data to assess service impact while results obtained serve as a guide to help improve organizational effectiveness and informs program decision.  

Our M & E goal is to help you eliminate waste, measure impact and to identify areas of strengths and needs. 


Social Service Workforce

 

 With an overwhelmed social service workforce, AOP strives to create professional level of social workers/ Para-professional social service workers at the community level. We work with social services organization locally and in developing countries by providing technical assistance, hands on coaching and mentoring to help strengthen organizational deficiencies and gaps. We will train your social workers to understand the rudiments of engagement needed to facilitate one on one conversation and also on a higher-level.  

Field Social workers will learn effective case management techniques; acquire skills needed to initiate referrals and complete acceptable investigations in a timely manner thereby preventing burn out. 


QI

 

We are solution oriented and constantly looking for ways to better processes. Our QI division will help your organization develop a quality improvement action plan that will ensure clear improvement goals, identify possible strategies and the specific intervention to follow to achieve desired goals.
